Nationwide Children’s Hospital Care Connection

Nationwide Children’s Hospital nurse practitioners and mental health professionals are now at Columbus City Preparatory School for Girls every Thursday to help keep your child healthy so she can succeed in school. In partnership with our school nurse, the health services offered provide quality physical and mental health care in a friendly and familiar school setting. Appointments are available during school hours.

Services offered by the nurse practitioner include:

* Sick appointments
* Physical exams
* Sports physicals
* Asthma therapy
* Teen health — Healthy weight, drug, alcohol and tobacco use prevention, and how to avoid teen pregnancy and sexually transmitted diseases, including HIV/AIDS

Services offered by the mental health professional include:

* Community-based counseling — Students facing emotional and behavioral difficulties are referred for in-school and in-home services. Students are assessed and may be referred to other NCH treatment programs as appropriate.
* School-wide prevention support
* 24/7 behavioral health crisis support – For clients enrolled in the program. For urgent needs, you should always first call your child’s pediatrician.

The child’s parent/guardian must provide consent for your child to be seen by the nurse practitioner and/or the mental health professional. Consent forms are available on the Columbus City Schools’ website at, for pick up at here at CCPSG or can be sent home with your child or by mail.

No child will be denied care due to inability to pay for services. Appointments are preferred, but not necessary. Students Receiving Special Education Services Progress Report Dates

Progress reports for students receiving special education services will be sent home eight times during the 2015-2016 school year. These progress reports directly address the goals of your child's Individualized Education Plan (IEP), and are different from the regular report card you also receive from your child's school. The dates are posted on the Columbus City Schools e-calendar that is on the district's website.

If you do not receive a specific IEP Progress Report for your student, please contact the principal. The principal will be happy to provide you with a copy.

Section 504 is a federal law designed to protect the rights of individuals with disabilities and requires public school districts to provide a free appropriate public education to each qualified student within the school district.
