Appointments are available during school hours.

Services offered by the nurse practitioner include:

* Sick appointments
* Physical exams
* Sports physicals
* Asthma therapy
* Teen health — Healthy weight, drug, alcohol and tobacco use prevention, and how to avoid teen pregnancy and sexually transmitted diseases, including HIV/AIDS

Services offered by the mental health professional include:

* Community-based counseling — Students facing emotional and behavioral difficulties are referred for in-school and in-home services. Students are assessed and may be referred to other NCH treatment programs as appropriate.
* School-wide prevention support
* 24/7 behavioral health crisis support – For clients enrolled in the program. For urgent needs, you should always first call your child’s pediatrician.

The child’s parent/guardian must provide consent for your child to be seen by the nurse practitioner and/or the mental health professional. Consent forms are available on the Columbus City Schools’ website at Columbus.gov/schoolbasedhealthservices, for pick up at here at CCPSG or can be sent home with your child or by mail.

Students Receiving Special Education Services Progress Report Dates

Progress reports for students receiving special education services will be sent home eight times during the 2015-2016 school year. These progress reports directly address the goals of your child's Individualized Education Plan (IEP), and are different from the regular report card you also receive from your child's school. The dates are posted on the Columbus City Schools e-calendar that is on the district's website.

If you do not receive a specific IEP Progress Report for your student, please contact the principal. The principal will be happy to provide you with a copy.
